Home School Policy

Parents wishing to home school their students must complete a
Home School Release Form

Forms must be submitted annually and signed by a parent or
guardian in front of a notary public. Forms should be turned in to
Uintah School District Office
635 West 200 South, in Vernal.

A notary public is available at the school district office.
Please bring a valid driver's license.
