Letter of Intent – Jan. 20, 2017 - due to principal/supervisor; Principals/supervisors, submit Letter of Intent forms and summary sheets to Lance Hatch by Jan. 27 , 2017. Information communicated in Letter of Intent is non-binding.

Student Boundary Change Requests – Feb. 17, 2017 - submit to Shannon Deets.

Incentive for Contracted Employees' Early (see date below), Written Notification of Intent to Resign at the End of the 2016-2017 School Year - Submit to Lance Hatch.
In order to be deemed eligible for the financial incentive of $500, an outgoing, contracted employee must submit a signed statement by the end of the work day on Friday, January 27, 2017, agreeing: to not voluntarily leave his or her position prior to the end of the 2016-2017 school year; and to resign effective at the end of the 2016-2017 school year.
Contracted employees who provide such written notice by the deadline specified above, will receive a financial incentive of $500, to be paid in the June 25, 2017 paycheck.
No financial incentive will be offered for outgoing, contracted employees who submit their written resignation after January 27, 2017.
Please note that any contracted employee who works a 180-day schedule and who fulfills the contract requirements (i.e., completes the contract year), will have benefits (including medical insurance, if applicable) extended throught August, 2017.

Lane Change Application - March 6, 2017 - submit to Lance Hatch.
Application is available at www.uintah.net/districtoffice/hrdocs. Please note that there is no guarantee that employees will be granted lane changes for 2017-2018. Please see policy 005.0525 Qualifications for Salary Lane Change for details.

Application for Educational Financial Assistance for Educators - September 1, 2017 - Submit to Lance Hatch
Financial assistance up to $2,000 potentially available for educators to complete additional education required in order to become state qualified or NCLB "highly qualified" for their assignments. You can access the application form at www.uintah.net/districtoffice/districtforms. Please see policy 005.5000 Reimbursement for Educational Expenses for details.

Educator License Upgrade and Renewal
Please visit http://www.schools.utah.gov/cert/License-Renewals.aspx to review the steps required by USOE. Every year a few USD educators miss the renewal/upgrade June 30 deadline because they don't have all of the steps completed by then, and they then must work to renew an expired license. Please make sure that doesn't happen to you.
Level I to Level II License Upgrade - Third-year Level I Teachers
a.
Monday, March 6, 2017 – Deadline for completing fingerprint process, online ethics review, and Praxis II Principals of Learning and Teaching (PLT) test (Fingerprint background check must clear before the license can be renewed, and clearance of fingerprint background checks sometimes takes several weeks. PLT results typically post on CACTUS 4-6 weeks after testing is completed);
b. Friday, April 7, 2017 – Portfolios should be ready for principal review; teachers schedule portfolio review session with principal, to take place no later than May 1, 2016;
c. Monday, May 1, 2017 –All portfolio reviews completed by principals; principals, please email Lance Hatch after completion of portfolio review sessions; Lance requests CACTUS license upgrade and emails third-year teachers to pay the last online fee to upgrade the license;
d. Suicide Prevention Training is required for license renewal. The next training will take place on February 7, 2017 at 4:00 p.m. at the district office. An additional training will take place on May 30, 2017. Please contact Shannon Deets with questions regarding the suicide prevention training.
Level II tor III Educator License Online Renewal - Level
II or III Teachers whose license expires June 30, 2017
a. Notices have been sent by USOE to all teachers whose Level II or III license expires on June 30, 2017.
b. Monday, March 6, 2017 – Recommended deadline for completing fingerprint process and online ethics review (Fingerprint background check must clear before the license can be renewed, and clearance of fingerprint background checks sometimes takes several weeks).
c. Friday, April 7, 2017 – Recommended deadline for having your administrator verify points earned by educator during the five-year licensing term (Please use Level 2/3 License Renewal Form , located at http://www.schools.utah.gov/cert/Forms/LevelTwoThreeRenewal.aspx (For Categories of Acceptable License Renewal Activities, please visit http://www.schools.utah.gov/cert/License-Renewals/Renewal-Information/ Categories-Acceptable-Activities.aspx )
d. License renewal should be completed online.

Longevity Steps
If you qualify for longevity steps, please notify the Human Resource Office in writing no later than February 1st of the year prior to becoming eligible for the longevity step.

If you have a change in family status at any time during the year, it could affect your benefits.
